Medical Package – Appointment Details

You have booked in for a Medical Package.

On your appointment day please bring the following documents:
1/ Driving License (Photocard)
2/ The Fee (£70) Payable via cash
3/ Please provide a copy of your Computerised Medical Summary. This can be obtained via your registered GP office.

Cancellation Policy.

All appointment booking fees are non-refundable. If you wish to amend your appointment this can only be done by giving a minimum 24 hours notice. If a minimum of 24 hours notice is not provided, a further booking fee will be required before another appointment can be booked.
